Sports Physicals 8/5/2008
Sports physicals will be offered to all athletes for the 08-09 school year at Quigley Catholic on Tuesday, August 5th at 5:00 PM. All athletes must have a completed physical examination before the first practice in order to participate. Cost is $10. Physical forms must be filled out prior to arrival for physical. You can pick up a form in the school office. A copy of your immunization record must accompany the form. Please call if you have any questions. | |
